Consent
All secondary schools in Scotland receive pre-printed consent packs that are ready to distribute to pupils in S1.
Each consent pack contains:
- a named invitation letter
- a consent form
- a leaflet with relevant information about the HPV vaccine for young people and their parents and carers
Blank consent packs are provided for use if a personalised form for a pupil is missing.
Both young people and their parents and carers are asked to provide consent to receive the HPV vaccine.
Consent for secondary school pupils
Pupils should be encouraged to talk to their parent or carer about getting the HPV vaccine first.
However, secondary school pupils may be vaccinated after giving their own consent following discussion with the vaccinator.
This is the case even if their consent form is not returned.
Learn more about consent for those under 16 years old on NHS inform (external website).
What to do with the consent packs
It is important that young people and their parents and carers receive and return the consent packs as soon as possible.
They should sign and return the consent form to school even if the young person is not going to have the vaccine.
You can remind pupils, parents and carers through school communications to return their completed consent form.
Your local NHS health board will arrange a date with you to collect the returned parental consent forms from your school.
Advise young people, parents and carers to contact their local NHS board if they:
- lose their consent pack and need a new one
- change their mind about consent and want to withdraw consent in writing
Consent form guidance
If a parent or carer is struggling to fill in the consent form, you might want to consider offering drop-in sessions to help.
A consent form guidance document is available for parents and carers. This explains how to fill in the consent form.
There is also an animation about consent for young people and vaccinations

About our resources
The information provided in the letter and leaflet has been tested with 11 to 13 year olds. It was also tested with parents of 11 to 13 year olds.
This was done to:
- ensure adequate understanding of the vaccine and its benefits
- enable informed uptake of the HPV vaccine
