“I am requesting the following information about cancer diagnosis:
- The number of recorded instances of an incorrect or missed cancer diagnosis.
By incorrect diagnosis I mean when a patient was eventually diagnosed but was initially given an incorrect diagnosis or their cancer was missed altogether, and that it was recorded or noted by health board or healthcare professional.
Please provide the figure for the last six financial years (2018/19, 2019/20, 2020/21, 2021/22, 2022/23) and last six calendar years (2018, 2019, 2020, 2021, 2022, 2023)
- The number of recorded instances of a terminal diagnosis of cancer. By terminal diagnosis I mean when it has been recorded that a patient’s cancer has been detected and diagnosed but said to be terminal.
Please provide the figure for the last six financial years (2018/19, 2019/20, 2020/21, 2021/22, 2022/23) and last six calendar years (2018, 2019, 2020, 2021, 2022, 2023).”
