“Please provide the appropriate PHS or NHS policy with regards to patients who may be changing health practices or even moving to a different region within Scotland and their rights - or otherwise - to continue with their existing medication.
This information should include the PHS/NHS policy on whether patients who are on existing (and successful) medications should be allowed to continue with that medication - or not, in which case please provide the policy/procedure with regards to the new practice reassessing the patient's medical condition(s) from scratch, thereby renewing the patient's prescriptions and their medications - or possibly even ceasing them altogether - as a result.
I believe there must be some sort of PHS/NHS continuation policy in this regard.”
