“I am writing under the Freedom of Information (Scotland) Act 2002 to request information regarding support provided to bereaved families following the death of a child or young person under the Child Death Review (CDR) process.

Specifically, I would like information covering the period from October 2021 (when the National Hub for Reviewing and Learning from the Deaths of Children and Young People was introduced) to the present. Please provide the following information:

  1. Number of Child Deaths Reviewed: The total number of child deaths (under 18s) reviewed by your board or organisation under the national Child Death Review process since October 2021.
  2. Provision of a Key Contact: Of those cases, how many bereaved families were:
  3. a) Assigned a designated key contact to support them before, during or after the CDR process? 
  4. b) Not assigned a key contact?
  5. Profession of Assigned Key Contacts: For families who were assigned a key contact, please provide a breakdown by profession of the individuals assigned (e.g. bereavement midwife, consultant, nurse, CDR coordinator, health visitor, family liaison office etc.).
  6. Timeliness of Contact: For each case where a key contact was assigned, please indicate (if recorded):
    • The average time between the child’s death and first contact with the family by the key contact.
    • The shortest and longest time recorded for initial contact.”
