About this release
This release by Public Health Scotland (PHS) provides a monthly update on the number of operations in NHSScotland that were cancelled the day before or on the day of treatment. Cancellation reasons are grouped into cancelled by the hospital for clinical reasons, cancelled by the patient, cancelled by the hospital due to capacity or non-clinical reasons, cancelled for other reasons, and cancelled for unknown reason.
Total cancelled planned operation figures in October 2025 were affected by an ongoing issue in NHS Grampian's Central Decontamination Unit (CDU) which led to a number of elective procedures being postponed, see supplementary data tables file 'Notes' tab for additional information.
Main points
During October 2025:
- There were 27,862 operations planned to take place across NHS Scotland. This is 12.2% higher (+3,038) than the number planned a year previously in October 2024 (24,824) and the highest number of operations planned since January 2020. Overall, the number of planned operations has been increasing since the COVID-19 pandemic impacted service provision, albeit with a levelling off in the 12-month rolling average between May 2024 and June 2025. In the latest 12 months, from November 2024 to October 2025, there was a 4.5% increase (+12,732) in the number of planned operations (297,014) compared to the previous 12 months (284,282). This remains 10.7% lower (-35,684) than those planned to take place in the 12 months from November 2018 to October 2019 (332,698), prior to when the pandemic began to severely impact service provision.
- 9.3% (2,601) of planned operations were cancelled the day before or on the day the patient was due to be treated in NHSScotland. This is higher than the rates reported during October 2024 (8.7%; 2,155) and October 2019 (8.5%; 2,468), which is the last October comparison time point before COVID-19 began to impact service provision. The 12-month rolling average of percentage of cancellations shows some decline from the start of 2023 and then a levelling off from the middle of 2024 onwards. Across NHS boards, the percentage of cancelled planned operations ranged from 5% to 21% in this month (the latter affected by the known issue in NHS Grampian).
- Of all planned operations during October 2025 in NHS Scotland, 783 (2.8%) were cancelled by the hospital for clinical reasons, 828 (3.0%) were cancelled by the patient, 894 (3.2%) were cancelled by the hospital due to capacity or non-clinical reasons, and 96 (0.3%) were cancelled for other reasons.
Please note that fluctuation in the trend in the monthly number of planned operations will be caused partly by variation in the number of working days per month, i.e. weekdays excluding public holidays, as theatre capacity tends to be lower at weekends and on public holidays.
